Coverage area: Sioux Falls, Tea, Harrisburg, Brandon, Hartford, Lennox, Crooks
Program service fees are not billed to insurance but may be covered by Health Savings Account funds. MDHC accepts most major insurance plans. Should you have any questions, please feel free to call the office at 605-250-1200.
Routine medical care visits and services are billed to your insurance as is customary. Patient responsibility costs will be billed to the patient as is required by the patient’s health plan.
Premium Plus Program
Premium Plus Program is available to a limited number of memberships per provider to ensure members requiceive the highest quality care and attention.
Pricing
$399 per adult per month or $3500/yr and $199 per month per child (0-18 yrs) or $2000/year.
Member Benefits
- Initial intake visit and physical exam that will give your provider the opportunity to learn about you, your health care needs and your health care goals.
- Unlimited 7-days per week direct text/phone access to your provider between the hours of 7:00 am and 10:00 pm. After-hours provider is available for urgent after-hours needs.
- No in-home visit travel fees for Premium Care Members.
- Guaranteed same day appointments (in-person, in-clinic, via video or via phone)
- Guaranteed same day call backs between the hours of 7:00 am and 10:00 pm 7 days per week.
- In home testing (COVID, Strep Throat, Influenza, RSV, and more).
- In-home lab specimen sample collection.
- Care management and coordination when specialty or inpatient services are required. Per your request your provider will attend specialty care appointments with you.
